I have done this and can fax you a form showing you what it looks like,
 H O W E V E R
It is not for the faint of heart
(you must use an HP LaserJet)
When I did it I didn't do it in SQR but in a very similar language.
The trick is to use HP's cursor addressing in a nested loop

You position the cursor to the upper left corner of the zero row of the first
column in the grid you want to fill.
Then move down the number of rows to reach the bubble you want to darken
and then print the HP char for a square (or Round) bullet.
after printing return to the target spot and step one column across in the
inner loop
and then drop down for the next digit or character etc.  Loop till you get to
the  end of the serial number.

NOTE if you are dealing with an NCS scanner and particularly if you are
dealing with a booklet or multipage form there is more involved and you
really need a full knowledge of SFW (Scantools for windows)  ncs's
proprietary programming language.
contact me off line if you would like me to send you a copy of a preprinted
form so you can see what you are getting into. If it is what you want, I can
send you the
loop code and the hp strings to rack the cursor to where you want it.

ONE OTHER THING. If it is for an NCS scanner, don't even think about printing
your own blank forms on a laser jet.  the registration, skunk marks zebra
marks etc are quite critical.  Use only an NCS authorized printer.

Printing the logo is easy if the printer has sufficient memory to hold it.
you simply download it at the top of the job. and invoke it.

Perhaps a better way is to do it in two passes, one where you are simply
printing
the logo using ms word or some such then, using the database to fill in the
bubbles.
