Generating a form to be scanned

Hi,
I am currently working on a project where student data by class attended
must be output to a report (form) that will eventually be scanned. Each
class must have a unique sequence number. The ovals (located at the top
right hand corner of the report) for that number must be pre-filled by the
report generator so that when the form is scanned, that number will be
picked up by the scanner. Example: if the first 2 numbers were 13, oval'd
1 in column 1 must be filled; oval'd 3 in column 2 must be filled.
In addition to that, another goodie: the form will most likely need to have
the UMASS logo on it.
The professors will then fill in the ovals for each student's grade and the
form will be scanned.
I have used SQR for loads but never reporting. However from what I know of
SQR reporting, it does not seem that SQR is the best solution for such a
complex report.
One option I am pondering is to produce the form separately, then run the
report using SQR with the forms in the printer box so that the report just
needs to print student/class data in the correct position on the form.
However this would require precision - (eg: being sure the ovals for the
unique number are filled in completely). So I am not sure this would work
(??)
I would love to hear from anyone who has done something similar (or has
worked with complex reports/forms). What did you use to produce the
report/form? What would you recommend?
Thank you in advance for any information you can share,
Hope you all had a good Thanksgiving break!
Anne-Marie
