Duplicate form and email it

Hello all,
I have a task to:       1. provide a screen for users to enter data that
will then be placed into a table
                                2. the procedure must create an email
attachment that duplicates an existing office form with all its different
fonts that has the entered data as part of the form
                                3. finally the form must be emailed to
various people.
I have created the data entry screens and stashed the data into a database.
Now I am stuck.  Where is the best place to look to find out how to create
the form and then email it as an attachment??
We are using:   SQL server 6.5 (we will be upgrading to 7.0 when Maximo
allows it)
                SQR 4.3
                Maximo 4.01
Any direction would be appreciated.
