Loading an image file under data

I have been asked the following question:
Is it possible to include a scanned .BMP of a form in your SQR, and print
over top of that .BMP?

Basically, they have a form with boxes to fill in data.  Is it possible to
print that form out (within the SQR), and then print data extracted from
within the SQR on top of it?
