HP Printer setup

I am having trouble routing my sqr reports to our HP LaserJet 5si
printer. When I set type=lp in the declare-printer section the report
seems to print correctly but when I set it as shown below my first
page does not have a heading and each page only prints 10-12 lines
starting lower on each succeeding page. Can anyone point me in the
right direction? Is there any good documentation for declaring
printers?
