sqr requrirements document

I am looking for a  document (or excel) for gathering end users requirements
creating sqrs (interfaces, updates to dbs, reports). If you could send
me an attachment of what you use for gathering end user requirements,
that would be helpful.we are looking at keeping this to help bridge
functional & technical resource documentation.
